Ordering an Information Packet

Due to residency requirements, the test packet for a new notary public commission must be sent to applicants at their home address (street address required). Tests are numbered and assigned to the applicant and are non-transferable.

To request a test packet, please provide the following:

  • Applicant’s name.
  • Applicant’s home address.
  • Post office box number (if applicable).
  • Telephone number (optional).
