Application for Initial Collection Agency Solicitor's Certificate Form (An application in no longer required. Submit an alphabetical list of solicitor's first and last names, any aliases used and their date of hire. Fee is $1.00 each.)

Branch Office Application Form (An application is no longer required. Submit the name, address, city, state, zip and phone number of the branch office. Fee is $50 per branch.)
